Once a Client completes and submits an Intake Form, it's automatically added to their record and can be accessed and referenced in several areas throughout your Professional Account.
To view an Intake Form in a Client's record, navigate to your:
- Business Profile
- Select Clients to open your list.
- Search for and select the Client to open their record.
- Select the Intake Forms tab.
- Select View to open the Intake Form window.
To view an Intake Form through an appointment on the Schedule, navigate to your:
- Business Profile.
- Select Schedule.
- Locate and select the Client's appointment block.
- Select details to open the appointment details.
- Contraindications and Medications are listed in a dropdown menu below the client's name for quick access. To view the full form, select the Intake Forms tab.
- Select View to open the Intake Form window.
To view a Client's Intake information through a Client's SOAP Notes history, navigate to your:
- Personal Profile.
- Select SOAP Notes.
- Type in Search Clients to locate and select a Client to open their history.
- Click on Contraindications or Medications to see a dropdown menu of the client's information.
