Below is a list of components available in the Forms & Documents, each component's available options, and information related to how components interact with a client's record.
Important note: Client Contact Details, Existing Conditions, and Medications all interact with the client record.
- If you would like for the client's name, phone number, address, etc. to be filled automatically on their record, be sure to use the Client Contact Details component.
- If you would like the client's existing conditions to appear within the Contraindications section of their client record, be sure to use the Existing Conditions component.
- If you would like the client's medications to appear within the Medications section of their client record, be sure to use the Medications component.

Pages
- The follow components can be added within a page and any number of pages can be added to your form
- Once a page has been added, they can be reordered by dragging and dropping the page

Columns & Rows
- This component is useful for creating columns where other components can be added. You can choose the number of columns and set the width for each column.
- Columns will stack when the screen size changes. So this is best to use if you would like the components added within the columns to adjust properly for use on a mobile device.
- To add more rows, you can drag and drop additional Columns & Rows components beneath the previously added column.

Table
- This component is useful for creating a table where other components can be added.
- The table will remain in the structure you set and will not adjust based on screen size or device type.
- When this component is added to your form, you will be able to choose the number of columns and rows you would like to add.

Heading & Free Text
- This component is useful for adding text to your form that is unrelated to a component. Such as headings of sections or adding waiver text.
- When this component is added to your form, you will be able to enter text and choose font size, styling, etc.

Client Contact Details
- This component is useful for gathering contact information from your client. Information entered into this component will be added to the related fields onto the client's record.
- When this component is added to your form, you will be able to select which fields within this component should be required to submit your form.

Short Answer
- This component adds a short text field and is useful for providing a field for the client to enter a short answer.
- When this component is added to your form, you can choose the title (like the question the client should answer), a hint which adds an example into the field, a tooltip which provides additional help, and sub-text which will appear below the field, and an option to make the field a requirement to submit the form.

Long Answer
- This component adds a long text field and is useful for providing a field for the client to enter a short answer.

Tags
- This component allows a client to easily add a list of items.
- When this component is added to your form, you can choose the title, hint, and tooltip

Checkbox
- This component allows a client to select a checkbox.
- When this component is added to your form, you can choose the title, hint, tooltip, and make it a requirement to submit the form.

Radio
- This component adds radio buttons where a client can select from a number of options.
- When this component is added to your form, you can choose the title, hint, tooltip, and make it a requirement to submit the form.
- Any number of radio options can be added and they can be aligned vertically or horizontally in your form.

Select
- This component adds a drop down list into your form.
- When this component is added to your form, you can specify all of the options that appear within the drop down. You can also specify if a selection is required to submit the form.

Date
- This component will allow a client to select a month, day, and year within drop down menus.
- When this component is added to your form, you can specify the text that appears above the month, day, and year drop downs, a hint, a tooltip, sub-text that appears below the date picker, and you can specify whether a date is required to submit the form.

Calendar
- This component will allow a client to select a date from within a calendar.
- When this component is added to your form, you can specify the text that appears above the date picker, a hint, a tooltip, sub-text that appears below the date picker, and you can specify whether a date is required to submit the form.

Client Signature
- This component allows a client to provide their signature.
- When this component is added to your form, you can specify the text that appears above the signature line, the text that appears directly below the signature line, a tooltip, and sub-text which will appear at the very bottom of the component. You can also specify whether a signature is required to submit the form.
Contraindications

Existing Conditions
- This component is similar to the checkbox component. However, if the client checks the box, the title associated with the checkbox will be added to the Contraindications section on the client's record.

Medications
- This component is similar to the tags component. However, when the client enters medication names, they will be listed within the Medications section on the client's record.

Markable Image
- This component will include an anatomical image. The client can select areas of the image and specify any pain points they may have.
- When this component is added to the form, you can specify the title, tooltip, sub-text, and you can specify whether the client is required to interact to submit the form.
