A Client's Intake Form can be accessed and completed through their record in the Client's section of your Business Profile.
To access an Intake Form using MassageBook's mobile desktop site, navigate to your:
- Business Profile
- Select Clients to open your list.
- Search for and select the Client to open their record. If the Client doesn't already have a record, choose Add Client instead, fill in their contact information, and select Save.
- Select the 3 dots located in the top right of the client's record.
- Select Intake.
- Select the form you wish to complete from the dropdown.
- Select Fill out on this device to open the Intake Form in a new tab.
If you are updating a previously completed form in office, you will need to navigate to your:
- Business profile
- Click Clients
- Search for and open the client's record (or add a new client)
- Select the Intake tab within the client record.
- Select to View the most recently completed form.
- Select the Edit icon at the top right of the form.
Once the form is filled in and saved, it's automatically added under the Intake Forms tab in the Client's record and stored there for viewing. If editing a previously completed form, the previous submission will remain unchanged and a new form submission will be created displaying the most recent edits to the form.
Notes:
- As an added security step, when the Intake Form opens in a new tab, you can close out the tab logged into your MassageBook account prior to giving the device to the client. This will restrict the clients access to only their Intake Form and will prevent any chance of the client viewing other information within your MassageBook account.
- This functionality is not yet available on the MassageBook Manager mobile app.
