Viewing a Completed Intake Form

Once a Client completes and submits an Intake Form, it's automatically added to their record and can be accessed and referenced in several areas throughout your Professional Account. 

To view an Intake Form in a Client's record, navigate to your: 

  1. Business Profile 
  2. Select Clients to open your list.
  3. Search for and select the Client to open their record. 
  4. Select Intake Form to expand the menu. 
  5. Select View to open the Intake Form window. 

To view an Intake Form through an appointment on the Schedule, navigate to your:

  1. Business Profile. 
  2. Select Schedule
  3. Locate and select the Client's appointment block to open the side menu. Note: C and M icons on the appointment block denote that the Client added contraindications and medications to the form. 
  4. Select Edit to open the appointment details. 
  5. Select Intake Form to open the Intake Form window. Note: Contraindications and Medications are listed in a dropdown menu to the left of the Intake Form button for quick access. 

To view an Intake Form through a Client's SOAP Notes history, navigate to your:

  1. Personal Profile. 
  2. Select SOAP Notes
  3. Type in Search Clients to locate and select a Client to open their history.
  4. Select Intake Form to expand the menu. 
  5. Select View to open the Intake Form window.
