Customizing the Intake Form Waiver

MassageBook's Intake Form Waiver includes default copy, but you have the ability to edit it if needed. 

To customize the Intake Form Waiver, navigate to your:

  1. Business Profile.
  2. Select Settings.
  3. Select Client Intake Form.
  4. Select Customize Your Intake Form to open the Intake Form window.
  5. Select the Waiver tab.
  6. Select within Client Waiver Form to place your cursor in the text editor. 
  7. Add and remove copy as needed. Note: Use the text editor toolbar to format and style the form. 
  8. Select Save Changes to save your progress. 
  9. Select X in the top right corner to close the window once your work is complete.

Note:

  • The Client Signature and Date fields are inactive when customizing the Intake Form. They can only be filled in when completing a Client's Intake Form.
