Once a Client completes and submits an Intake Form, it's automatically added to their record and can be accessed via the Intake Forms tab in their record. If the client has submitted a form, but you as the provider need to make adjustments to their submitted form, you can do so by following the steps below:
- Navigate to the desired client's record.
- Select the Intake Forms tab.
- Select to View the form you wish to edit.
- Once the form has opened, select the Edit icon located in the top right corner of the form:
Note: Editing a completed form will create a new form submission in the client's Intake Form tab time stamped with the date the changes were made. For example, the form submitted on 3/31 was edited by a service provider on 5/17:
